Choice Ropers Giveaway Rules
1. NO PURCHASE IS NECESSARY
Limit one entry form per household. To enter without purchasing,
mail a 3-inch by 5-inch index card indicating your request to
enter the drawing and containing your full legal name, address,
telephone number, and email address to: Professional’s
Choice Roper's Giveaway, Professional’s Choice Sports Medicine
Products, Inc., 2709 Via Orange Way, Spring Valley, California
91978.
2. HOW TO ENTER
Complete and return all entry forms to: Professional’s Choice
Ropers Giveaway, Professional’s Choice Sports Medicine Products,
Inc., 2709 Via Orange Way, Spring Valley, California 91978. Limit
one entry form per household. All entries must be received by 5:00
p.m. Pacific Standard Time on December 31, 2003. Entry forms may
not be submitted via facsimile or electronic mail.
3. ELIGIBILITY
Entry forms are offered only in the United States (including Alaska
and Hawaii) to legal residents of the U.S. who are licensed drivers,
18 years of age or older, except for employees (and the immediate
families and members of the same household of such employees)
of Professional’s Choice Sports Medicine Products, Inc.,
it's affiliated companies, their respective divisions, affiliates,
dealers, and subsidiaries, who may not enter the drawing. Void
in Puerto Rico, outside the United States, and where prohibited
or restricted by law.
4. PRIZES / APPROXIMATE RETAIL VALUE ("ARV")
One Grand Prize includes approximately 11 prizes and has an estimated
retail value of $ 4,150. One Second Prize includes approximately
4 prizes and has an estimated value of $724. One Third Prize
includes approximately 4 prizes and has an estimated value of
$474. One Fourth Prize includes approximately 4 prizes and has
an estimated value of $374. One Fifth Prize includes approximately
4 prizes and has an estimated value of $324. All winners are
responsible for all taxes (federal, state and/or local), freight/delivery
fees and any other expenses not specifically listed which relate
to acceptance and use of the prizes. All travel (airfare and
hotel) prizes are restricted to certain airlines and hotels,
class of service, and travel dates, and must depart and return
from the same location in the United States. All other travel-related
expenses, such as, but not limited to, food, taxes, gratuities,
ground transportation, and parking, are not included. Prizes
may not be redeemed for cash.
5. PRIZE WINNER SELECTION:
Winners of free product are determined at random. Odds of winning
a prize are estimated at 1 in 200,000. Winners shall be chosen
via a drawing to be held in San Diego, California on February
4, 2003.
6. CONDITIONS
Professional’s Choice Sports Medicine Products, Inc. is not
responsible for entry forms or mail that are late, lost, misdirected,
damaged, incomplete, illegible or postage-due. By submitting an
entry form, participants (a) agree to these official rules; (b)
release Professional’s Choice Sports Medicine Products, Inc.,
its affiliated companies, their respective divisions, affiliates,
dealers, and subsidiaries, their respective directors, officers
and employees from any and all liability for any injuries, losses
or damages of any kind caused by their participation or resulting
from acceptance, possession, use, or misuse of prize(s); (c) acknowledge
that said parties have neither made nor are in any manner responsible
or liable for any warranty, representation or guarantee, expressed
or implied, in fact or in law, relative to the any awarded prize(s);
and (d) expressly grant to Professional’s Choice Sports Medicine
Products, Inc., its affiliated companies, their respective divisions,
affiliates, dealers, and subsidiaries, their respective directors,
officers and employees, the right to use their name, signature,
likeness, and photograph in connection with any and all advertising
activities. Prizes not redeemed by May 1, 2004 are cancelled and
void.
7. WINNERS LIST/RULES
For names of winners or a copy of the official rules, mail a stamped,
self-addressed envelope to: Professional’s
Choice Sports Medicine Products, Inc., 2709 Via Orange Way, Spring
Valley,
California 91978. Please specify "winners list" or "rules" on
the envelope. Only one request per envelope.
